Should the owner be present when registering a business name? 

  • When registering a business name in the Western Province, it is not mandatory for the owner to be present in person.

    However, to facilitate the process, you should take care of the following points:

    If the owner is unable to come, the application can be handed over to someone else. However:

    The owner must have correctly completed and signed the relevant application form. A certified copy of the owner's National Identity Card must be submitted.

    The person carrying the application must present his/her identity card to prove his/her identity.

How to get the Rs. 45000/= Nutritional Allowance for Pregnant Mothers?

  • To get the Nutritional Allowance for Pregnant Mothers, you should register at the nearest maternity clinic within the first 3 months of pregnancy and fill out and submit an application through the Grama Niladhari or the Early Childhood Development Officer.

  • How to get:

    Eligibility: Must be a pregnant mother registered at a maternity clinic.

    Application: Applications should be obtained and filled through the Early Childhood Development Officer or the Grama Niladhari of the Divisional Secretariat.

    Required documents: National Identity Card (NIC), Maternity Clinic Book, and Grama Niladhari Certificate.

Who is eligible for educational assistance?

  • School and university students from low-income families, with special needs, who have lost their parents/disabled can receive educational assistance in Sri Lanka. This includes educational equipment or monthly scholarships of up to Rs. 10,000 through Samurdhi Sipdora, the President's Fund and Divisional Secretariats

  •  

    Those eligible for educational assistance:

    Low-income families: Children from families with a monthly income of less than Rs. 6,000.

    Children with special needs: Children with disabilities or children with disabled guardians.

    Disaster victims: School students whose education has been disrupted due to natural disasters or accidents.

    Parents/disabled children: Students whose father has died or is suffering from an incurable disability.

    University students: Monthly assistance for undergraduates (e.g. Rs. 1500).

    Application methods:

    Social Services Division of the Divisional Secretariat.

    Through the Grama Niladhari.

How to register an NGO? 

  • It must be done through the National Secretariat for NGOs. For this, the registration certificate is issued after completing the required application form, obtaining the documents including the constitution, action plan and financial reports and obtaining the Clearance Reports from the Ministries of Defense and Foreign Affairs

What are the qualifications required to receive elderly assistance?

  • To receive elderly assistance in Sri Lanka, one must be over 70 years of age and have a low income or no permanent source of income. This

    can be obtained through the Divisional Secretariat and is mainly available to economically distressed adults who are not Samurdhi beneficiaries.

  • Basic qualifications required:

    Age: Must be 70 years of age or older.

    Income level: Must have no fixed monthly income or be on a very low income.

    Welfare status: Those who are not currently receiving Samurdhi allowances or other regular government pensions.

    Residence: Must be a Sri Lankan resident of the relevant Divisional Secretariat Division.

    Abandoned/Destitute: Isolated adults without children or sources of income

How to obtain copies of birth, marriage or death certificates? 

• Copies of birth certificates should be obtained from the Divisional Secretariat of the place of birth.

  • If the relevant certificate is in the database, you can obtain certified copies of it from any Divisional Secretariat office nearest to you.

  • Copies of certificates from 1960.01.01 to 2020.12.31 can be obtained through the system. (You can check and confirm whether it is in the database from the nearest Divisional Secretariat office.)

  • The required application form can be obtained from the District Registrar Division of any Divisional Secretariat off  

Fees charged,

  • When the birth certificate number and date of registration are known - Fee Rs. 120.00 per copy
  • If the birth certificate number and date of registration are not known, (as a search for documents not exceeding two years is required) - Fee Rs. 250.00 per copy 
  • The relevant fee can be paid to the Divisional Secretariat or to the Registrar General's Bank of Ceylon (Battaramulla Branch) account number 7039827, and the receipt (Bank Slip) provided there and the completed application form can be submitted to the relevant Divisional Secretariat and copies of certificates can be requested. Payments made through automated teller machines or online payments will not be accepted. 
  •  If the birth certificate is to be sent by post, a self-addressed stamped envelope should be submitted along with the application form and the receipt of payment as stated above. 
  •  If the birth has been registered, the number of copies of the relevant birth certificate requested will be issued according to the amount paid and if the birth has not been registered, a notification will be sent via Form B38.

How to register a birth? 

  • To register a normal birth, you should meet with a birth registration registrar in the relevant division three months before the birth and register the birth.

  • To register a birth after three months, meet with the Additional District Registrar and obtain the relevant information.

How to make amendments to a birth certificate?

• In a birth registration record,
 Details in Column 01 - Date and Place of Birth
 In case a name is not included in Column 02, to add a name or to change a name included in a birth certificate later

 Details in Column 04 - Father's details (except caste)
 Details in Column 05 - All mother's details
 Details in Column 06 - Parents' marital/singleness

• The relevant declaration form should be submitted to the Additional District Registrar of the Divisional Secretariat where the birth took place.
• The declaration form should be submitted by,
 The person who owns the birth certificate
 The father or mother
 The legal guardian
 A person who is dissatisfied with any detail in the birth certificate
• The fee for the declaration form is Rs. 60.00.
• A certified copy of the birth certificate that is to be amended must be attached to the declaration form. Written evidence must be submitted with it to prove the authenticity of the request.
• Some of the written evidence that can be obtained to prove the authenticity of the request made in the declaration form is given below for your convenience.
 A certified copy of the parents’ marriage certificate
 Children’s register/sibling register
 Certified copies of the father’s and mother’s birth certificates
 A certified copy of the birth certificate of a child born after the registration of the parents’ marriage
 In the absence of a child born after the registration of the marriage, a certified copy of the birth certificate of the eldest or youngest child of the subject child
 Certified copies of the records in the hospitals, maternity homes or copies of the midwife’s records
 Student’s work report, extract from the children’s admission register
 A copy of the electoral roll containing the names of the subject and parents
 A certified copy of the father’s death certificate if he is deceased

How to make widow's orphan's pension after the death of a civil pensioner?

• First, obtain the pensioner's death certificate and stop the pension.
• If there is excess money, write to the bank to get it.
• Then inform the relevant person to bring the following documents.
 Original copy of the pensioner's death certificate
 Original copy of the widow's birth certificate
 Original copy of the marriage certificate
 A photocopy of the widow's bank book
 A photocopy of the widow's identity card
 A photocopy of the widow's orphan's card or widow's orphan's number
 A passport-sized color photograph certified by the Grama Niladhari
 All photocopies of the above documents should be certified by the Grama Niladhari.
• After they provide the relevant information, take the file and take the necessary steps to create the widow's and orphan's pension.

 

Why have pensioners' pensions been stopped?

  • Stopping the pension due to non-submission of the life certificate for the relevant year.
